DR. CONSTANTINO Y. AMORES is clinical director of Neuro-Ortho-Trauma with the Charleston Area Medical Center in Charleston, West Virginia. He also is a clinical professor for the College of Medicine in the Department of Neurosurgery at West Virginia University - Charleston Division. Dr. Amores is a member of the Kanawha County Medical Society, the West Virginia State Medical Association, The Philippine Medical Association of West Virginia, the American Medical Association, the Organization of State Medical Association (president), the American College of Surgeons, the International College of Surgeons, The American Association of Neurological Surgeons, The Congress of Neurological Surgeons of America, the West Virginia Neurological Society, the Neurological Society of the Virginias and is the chairman of the West Virginia State Medical Association Delegation to the American Medical Association. He has written many publications and presented numerous lectures in the West Virginia area. Dr. Amores is a fellow of the American Board of Neurological Surgery, the American College of Surgeons and the American Board of Independent Medical Examiners. He earned undergraduate degree from the University of Can Carlos and his Doctor of Medicine, with honors, from the University of Santo Tomas in Manila, Philippines. DR. RICHARD S. KAPLAN is a physical rehabilitation and disability assessment expert in active practice for Rehab Analytics in the Monroeville, PA and Uniontown, PA area. Dr. Kaplan was trained in physical medicine and rehabilitation at Mayo Clinic in Rochester, Minnesota, after receiving his Doctor of Medicine, with research distinction, from University of Miami, in Miami, Florida. He is certified by the American Board of Physical Medicine and Rehabilitation with Pain Management Credentials from the American Academy of Pain Management. Dr. Kaplan is a member of the American Academy of Physical Medicine and Rehabilitation, and an oral examiner for the American Board of Physical Medicine and Rehabilitation. He founded his private practice of physical medicine and rehabilitation in 1995, and has continued to maintain an active and diverse practice of musculoskeletal medicine and pain management. Dr. Kaplan serves as a disability or impairment evaluator for multiple governmental agencies in several states, including Pennsylvania, West Virginia and Ohio. He has published numerous articles and presentations on issues related to rehabilitation and disability assessment. Currently, Dr. Kaplan also serves as medical director of the Inpatient Rehabilitation Unit at Uniontown Hospital, in Uniontown, PA. He also has extensive experience serving as a peer reviewer, utilization reviewer, and expert witness in a wide variety of civil and criminal matters by request of both plaintiff and defense counsel. LISA L. LILLY is of counsel with Maccorkle, Lavender, Casey & Sweeney, PLLC, where her main areas of practice are professional liability defense, professional licensure, healthcare risk management, products liability and general defense litigation. She has participated in/conducted hundreds of depositions, including plaintiffs, defendant fact witnesses, doctors, economists, other experts of all kinds, agents, claims personnel and corporate designated representatives. Ms. Lilly is admitted to practice before the West Virginia State Courts; the U.S. District Court of West Virginia, Northern and Southern districts; and the U.S. Court of Appeals for the Fourth Circuit. She earned her B.S. degree in nursing from West Virginia University School of Nursing and her J.D. degree from West Virginia University College of Law. STEPHEN C. LITTLEPAGE is a sole practitioner in Point Pleasant, West Virginia, operating the law firm of Hyer & Littlepage, where he practices in the areas of personal injury and criminal litigation. He has been a guest speaker for the WVTLA and Mountain State Bar Association on personal injury practice and paralegal training, as well as having served for a number of years as an adjunct professor for the Marshall University Community College. Mr. Littlepage earned his B.A. degree from West Virginia University and his J.D. degree from West Virginia University College of Law. He is a member of The West Virginia State Bar and the West Virginia Association of Justice. DR. ROBERT W. LOWE is in private practice and works with the Scott Orthopedic Center in Huntington, West Virginia. He previously served as a Major for the US Army at William Beaumont General Hospital in El Paso, Texas. Dr. Lowe also is a former clinical professor of orthopedic surgery at the Marshall University School of Medicine. He is a member of the American Academy of Orthopedic Surgeons, the American College of Surgeons, the North American Spine Society, the American College of Spine Surgery, the American Board of Spine Surgery, the West Virginia State Medical Society, the Cabell County Medical Society, the Vanderbilt Orthopedic Society and is the current president of the Tri-State Orthopedic Society. Dr. Lowe earned his B.S. degree from Morehead State University in Morehead, Kentucky. He completed his surgical internship, first year general residency, resident in orthopedic surgery and earned his Doctor of Medicine from Vanderbilt University. KYLE SMITH is a disability consultant/legal nurse consultant for Disability Associates and the Law Offices of R. Stephen Jarrell and L. Victor Navy, Attorneys-At-Law. He operates his own incorporated business representing clients, in hand with attorneys, regarding Title II and Title XVI claims for Social Security benefits. Mr. Smith is a former emergency department registered nurse with Charleston Area Medical Center. He is a certified state of West Virginia office of medical examiner and certified death investigator. Mr. Smith spoke at a seminar at the 119th annual meeting of the West Virginia Bar Association on "Trends in Legal Nurse Consulting." He is a member of the American Association of Legal Nurse Consultants, the National Alliance of Certified Legal Nurse Consultants and the West Virginia Bar Association. He earned his B.A. degree in the Regent's Program and his Associate of Applied Science in Paramedic Science from Marshall University in Huntington, West Virginia. DR. RALPH S. SMITH JR. is a board certified forensic psychiatrist and president of CPG, Inc. He is also board certified in psychiatry, child psychiatry, addiction psychiatry and medical management. Dr. Smith has performed over 10,000 independent medical examinations. He provides forensic psychiatric and psychological evaluations and litigation support for civil litigation, workers' compensation and criminal cases. Dr. Smith has been performing such evaluations for over 30 years and has extensive trial experience. He accepts cases statewide, regionally and nationally from plaintiff and defense attorneys, judges, and insurance companies. He teaches forensic psychiatry and addiction psychiatry to residents at WVU Department of Behavioral Medicine and Psychiatry. Dr. Smith is a Brigadier General (Ret.) USAF and former USAF psychiatrist and flight surgeon. He also is a former faculty member of the American Board of Independent Medical Examiners and American Academy of Disability Evaluating Physicians. Dr. Smith is the former medical director of West Virginia Workers' Compensation Division and Wells Fargo Disability Management. He earned his B.A. degree from West Virginia University, his MD degree from Northwestern University, his Master of Business Administration from the University of Charleston and his MA degree in communication studies from West Virginia University. Dr. Smith completed his medical internship at Evanston Hospital in Evanston, Illinois, and his psychiatry and child psychiatry training at the University of Cincinnati. MIRIAM E. WALLER is a legal nurse consultant in Huntington, West Virginia. Her specialty is in pulmonary and med/surg nursing applicable to personal injury and medical malpractice cases, and is fully acquainted with regulatory requirements and Joint Commission compliance standards applicable to the health care facility. Ms. Waller is currently a team leader, staff floor nurse with Hospice House in Huntington, West Virginia.
