DR. JAN C. BAX is a specialist in hand and microvascular surgery with Hand to Hand Defense, S.C., where he provides IME and second opinion and expert witness services. He has been a frequent lecturer with numerous publications throughout his career. Dr. Bax is a member of the American Society for Surgery on the Hand (fellow), American Association for Hand Surgery, American Academy of Orthopedic Surgeons (fellow), American Society for Reconstructive Microsurgery, Outagamie County Medical Society, Wisconsin Medical Society and Valley Independent Physicians. He earned his B.S. degree from Purdue University and his Doctor of Medicine from Indiana University School of Medicine. DR. JEFFREY CRONK is the owner of National Injury Diagnostics. He is the director of Education and Professional Development Spinal Kinetics. Dr. Cronk is a Board Certified Independent Chiropractic Examiner by The American Board of Independent Medical Examiners. His certification allows for the performance of Peer Reviews, Utilization Reviews and Impendent Chiropractic Examinations with the credibility established through the Board Certification. Dr. Cronk also received the Impairment Rating Certification by the North American Academy of Impairment Rating Physicians. He is a frequent lecturer with several publications in the chiropractic field. Dr. Cronk earned his Business and Pre-Chiropractic degrees from the University of Wisconsin, and his B.S and Doctorate of Chiropractic degrees from Palmer School of Chiropractic. DR. DAVID A. FETTER has an active orthopedic practice in the states of Wisconsin and Illinois. Dr. Fetter is a highly experienced practicing orthopedic surgeon for more than 33 years, and has evaluated and treated thousands of patients during this time. He is certified by the American Board of Orthopaedic Surgery, with an additional certification by the American Academy of Disability Evaluating Physicians in the Evaluation of Disability and Impairment Rating. Dr. Fetter has the distinct honor of being the only orthopedic surgeon licensed in the state of Wisconsin with certifications both by the American Board of Orthopaedic Surgery and the American Academy of Disability Evaluating Physicians. He is a fellow of the American Academy of Orthopaedic Surgeons and The Academy of Disability Evaluating Physicians. In addition to Dr. Fetter's active orthopedic practice, he performs Independent Medical Evaluations, Peer Reviews, and Expert Witness Testimony for both plaintiff and defense firms. His special interest is in the causation of complex orthopedic cases. Dr. Fetter has written multiple book reviews for physician education and training purposes in regard to the "Spine, the Guides Casebook" that accompanies the 6th Edition AMA Guides to the Evaluation of Permanent Impairment, and currently has been requested by the AMA to review the Second Edition of the Physician's Guide to Return to Work to be published in June 2011. MICHAEL A. MESIROW is a shareholder in the Milwaukee office of Kasdorf, Lewis & Swietlik, where his practice focuses on civil litigation. Mr. Mesirow is a member of the State Bar of Wisconsin, the Milwaukee Bar Association, the Civil Trial Counsel of Wisconsin and the Defense Research Institute. He has presented at many seminars, discussing various aspects of the defense of personal injury claims. Mr. Mesirow earned his B.A. degree from the University of Wisconsin and his J.D. degree from Marquette University Law School. DR. SARA J. SWANSON is a professor of neurology at the Medical College of Wisconsin and interim chief of the Division of Neuropsychology. She earned her doctorate from Washington State University, interned at the University of Washington School of Medicine and completed postdoctoral training at the Medical College of Wisconsin. Dr. Swanson is board certified in clinical neuropsychology and serves as a board examiner for the national oral examinations. She is active clinically and in the teaching and training of medical residents, graduate students and post doctoral residents in clinical neuropsychology. Dr. Swanson works as a medical consultant to disability insurance companies and conducts medico legal neuropsychological evaluations on individuals who have sustained traumatic brain injuries. She works in the Traumatic Brain Injury clinical at Froedtert and Medical College of Wisconsin following patients in the acute phase after head trauma. SUZANNE L. WARD is a legal nurse with iNurse Consulting, LLC and case manger with Humana. She has been an expert witness, and has court testimony experience and deposition experience. Ms. Ward is an adjunct faculty member on adult education programs with Concordia University of Wisconsin and Cardinal Stritch University. She is a frequent lecturer with numerous publications on nursing and legal nurse consulting. Ms. Ward is a member of the American Nurses Association, Wisconsin Nurses Association and American Association of Legal Nurse Consultants. She has received certificate as a case manager and legal nurse consultant. Ms. Ward graduated with an Associate, B.S. and Master of Science degrees.
