JOSHUA A. CARR, D.C., is a summa cum laude graduate of Palmer College of Chiropractic, the most prestigious chiropractic school in the world. He is the chief operations officer and sits on the board of directors of Mt. Gibborim, PLLC, the parent company of Coral Canyon Chiropractic He is devoted to raising the standard in health and wellness. Dr. Carr developed an intimate interest in musculoskeletal rehabilitation after he and his family were hit by a drunk driver. He was selected to receive more extensive training as a sports injury and rehabilitation intern under olympic bobsled trainer and rehabilitation expert, Dave Juehring, D.C., D.A.C.R.B., C.C.S.P., C.S.C.S. Having excelled in the program, Dr. Carr was awarded the Clinical Excellence Award in rehabilitation and sports injury. Before opening Coral Canyon Chiropractic, he practiced in Las Vegas as an associate physician under Dr. Ian Yamane, a member of the Chiropractic Physician's Board of Nevada and director of one of the top five personal injury clinics in the state. Through his experience in physiotherapy and personal injury, Dr. Carr feels that he can better help those that have been injured, and at times devastated, by the effects of a car crash. A unique and distinguished accomplishment for a doctor of chiropractic, he has been certified by the American Board of Independent Medical Examiners (A.B.I.M.E.) and is a graduate of the San Diego Spine Research Institute's whiplash and brain injury traumatology program. Currently, Dr. Carr continues to practice at Coral Canyon Chiropractic in Hurricane, Utah, the gateway to Zion National Park. NIKKI CHAMBERS is a legal nurse consultant with Chambers Medical-Legal Consulting, LLC, in Riverton, Utah, where she is the owner and CEO. She provides litigation support behind the scenes, for both plaintiff and defense. Ms. Chambers presently is a BLS instructor, an ACLS instructor and previously was a critical care lab instructor at Brigham Young University. Her memberships include the Department of Occupational & Professional Licensing, Nurse Diversion Committee, Magnet Steering Committee, and the National Nursing Education Initiative Committee. Ms. Chambers belongs to the American Association of Critical Care Nurses, the American Association of Legal Nurse Consultants and the American Nephrology Nurses Association. She earned her BSN degree from Brigham Young University, and her MSN degree, along with her post masters certificate in business and post masters certificate in teaching nursing, from the University of Utah. DOUGLAS C. FULLER M.D. is the medical director of the Hill Air Force Base Occupational Medicine Clinic, providing a full range of occupational medicine services for over 10,444 government service employees, 1544 Reservists, 840 Tenants and 4,800 active duty Air Force members, in a highly technical aerospace industrial atmosphere. The base actively manages the repair and maintenance of A-10, F-16 and F-22 Fighters, B-2 Bombers, Cruise Missiles, and C-130 Cargo aircrafts. The clinic staff of 18, including two physicians, one physician's assistant and four nurses, cares for workers during 11,000 annual visits for injuries, illnesses and occupational labs and exams. Dr. Fuller currently is adjunct faculty at USAF School of Aerospace Medicine in Occupational Medicine Residency in Brooks AFB, Texas and University of Utah School of Medicine in the Occupational Medicine Residency Program in Salt Lake City, Utah. His previous professional office appointments include: treasurer of the Residents and Recent Graduates (RRG) Section; RRG Section Resident representative to the American Medical Association (AMA) Congress of Delegates; chair-elect, RRG Section; and chair, RRG Section. Dr. Fuller is a member of the American Academy of Family Physicians, the U.S. Chapter Academy of Family Practice, Aerospace Medical Association and the American College of Occupational and Environmental Medicine (ACOEM). He earned is B.S. degree from the University of Utah, and his M.D. and M.P.H. degrees from the University of Utah School of Medicine. SAM GOLDSTEIN, Ph.D., is an assistant clinical professor at the University of Utah School of Medicine. He is the clinical director of the Neurology, Learning and Behavior Center in Salt lake City, Utah. Dr. Goldstein has authored thirty-four books, three clinical tests, dozens of chapters and peer reviewed research articles. He serves as editor-in-chief of the Journal of Attention Disorders and is co-editor-in-chief of the Encyclopedia of Child Development and Behavior. Dr. Goldstein completes approximately eighty forensic evaluations per year, primarily in civil litigation matters. He is an experienced expert witness with testimony in state and federal courts. TONNA M. JACKSON is a legal nurse consultant with Jackson Legal Nurse Consultants in Kaysville, Utah. She has many years of clinical experience and is the main caregiver for her 19-year-old step-son who, at 8 years of age had a traumatic brain injury. Ms. Jackson earned her degree from Ricks College and has had experience as a Spanish WIC nurse and as a diabetes education nurse. PETER R. SILVERO M.D. is board certified by the American Board of Orthopaedic Surgeons and specializes in the treatment of shoulder and elbow conditions. He graduated from the Uniformed Services University in Bethesda, Maryland and completed his residency training with the Air Force. Dr. Silvero later returned to the University of Texas Health Science Center to complete the prestigious Charles Rockwood Shoulder and Elbow Fellowship. He recently finished a distinguished Air Force career. After working in the space program for several years, Dr. Silvero completed his medical training and then served as a special operations flight surgeon. As an orthopaedic surgeon he gained considerable experience in sports medicine, total joint replacement and orthopaedic trauma. Throughout his career, Dr. Silvero has been deployed to the Middle East, Korea, and South America. He provides comprehensive orthopaedic care, including sports medicine and total joint replacement. Dr. Silvero also is a member of the American Academy of Orthopaedic Surgeons, Society of Military Orthopaedic Surgeons and the California Orthopaedic Association.
