DAVID BOYCE is an assistant professor of physical therapy at Bellarmine University and owner of Physical Therapy Plus. He has advanced training in orthopedic manipulative therapy and is author of the book Orthopedic Physical Therapy Secrets. Dr. Boyce is dual board certified in orthopedic and electrophysiologic physical therapy. He is also the only USA certified cycling coach in the Louisville area. Dr. Boyce specializes in the treatment of shoulder, cervical spine, knee and foot injuries, especially as they relate to swimming, cycling and running. STEVEN A. CARTER, D.C., has been in private practice with Carter Chiropractic in Louisville since 1985. He earned his doctor of chiropractic degree from Palmer College of Chiropractic, and his bachelor of arts degree from Western Kentucky University. Dr. Carter received his certification in peer review, insurance consulting, and independent chiropractic examination, from the Logan College of Chiropractic Campus in St. Louis, Missouri. He has instructed the program "Herniated Discs: New Guidelines & Language - The Role of MRI in Bodily Injury," sponsored by the Louisville Bar Association in November 2009. Dr. Carter serves on the Aetna Credentialing Committee and the Utilization Review Committee, Louisville, Kentucky. RICHARD T. HOLT, M.D., is a noted spine surgeon practicing with Spine Surgery, P.S.C., in Louisville, Kentucky. His practice serves as a regional referral center for complex spine reconstructive cases. He performs more than 400 instrumented spine surgeries annually. Dr. Holt specializes in adult and pediatric deformity, adult degenerative disc disease, non-fusion technologies, fractures, and tumors. He has published more than 25 publications, in addition to numerous other articles and book chapters concerning spine surgery, and he is a frequently invited speaker at spine surgery seminars around the world. Through the years, Dr. Holt has served on the faculties of the University of Louisville, the University of Kentucky, and Tulane University in New Orleans. He currently directs a training program in spine surgery and his fellows for the last 26 years now practice worldwide. Dr. Holt is an American Board of Orthopedic Surgery board certified spine surgeon and an American Board of Spine Surgery Diplomate. He received his undergraduate degree from the University of Tennessee, Knoxville, and his medical education from the University of Tennessee, Memphis. Dr. Holt is a member of numerous medical societies including the American College of Spine Surgery, the American Medical Association, the Clinical Orthopaedic Society, the Kentucky Medical Society, the Kentucky Orthopaedic Society, the Kentucky State Medical Association, the Scoliosis Research Society, the Spine Society of Australia, and the North American Spine Society. TONYA NEWTON graduated with honors from the University of Louisville with a B.S. degree in nursing. She is a board certified emergency nurse, and is a member of Sigma Theta Tau, the National Honor Society for Nurses. She has 17 years experience in emergency services. Ms. Newton was honorably discharged as captain after serving six years in the United States Air Force at Wright-Patterson Air Force Base, Ohio. In 2002, she was named Top Nurse Educator by Wright State University Emergency Residents. She is an advanced cardiac life support educator, and holds certifications in trauma nurse core course, pediatric advanced life support, basic cardiopulmonary resuscitation, combat casualty care course, and legal nurse consulting. Her nursing experience also includes cardio-thoracic intensive care, medical-surgical, hemodialysis, and intermediate care. Currently, she is a clinical instructor for Bellarmine University-Lansing School of Nursing. Ms. Newton continuously maintains her emergency nursing skills at Norton Healthcare Emergency Departments. She is the founding member and CEO of Highland Legal Nurse Consulting, LLC, where she assists attorneys with reviews for merit, medical chart analysis, demonstrative evidence, and depositions. MODERATOR - J.D. RAINE JR. is a member of the Louisville law firm of Ferreri & Fogle. Mr. Raine practices in civil litigation, personal injury defense, insurance, and equine law. Mr. Raine also is a certified mediator. He served as an assistant Jefferson County attorney from 1986 to 1992. Mr. Raine is a member of the Louisville (former member, Professional Responsibility Committee) and Kentucky (member, Equine Law Section) bar associations. Mr. Raine earned his B.A. degree from Washington & Lee University and his J.D. degree from the University of Louisville. CYNTHIA SMITH, RN, CLNC, is the owner of Smith & Associates Medical Consulting, LLC, in Louisville. She has more than 18 years of varied medical experience, including pediatrics, corrections, substance abuse, disease management, and utilization review. Her legal nurse experience includes medical malpractice, personal injury, and pharmaceutical cases. Ms. Smith earned her licensed practical nurse diploma and her associate degree in nursing from Spencerian College in Louisville. She earned her certification of legal nurse consulting from Vickie Milazzo Institute in Houston, Texas.
