DR. ELAINE M. BUKOWSKI is adjunct faculty at Sullivan University in Louisville. She earned her B.S. degree from State University of New York at Buffalo; her M.D. degree from State University of New York at Buffalo School of Medicine; and her J.D. degree from Louis D. Brandeis School of Law, University of Louisville. Dr. Bukowski is a member of the Kentucky Bar Association, as well as the American Bar Association. She is also a member of The American College of Legal Medicine, American Society for Clinical Pathology, American Medical Association, American Society of Anesthesiologists, Kentucky Medical Association and Jefferson County Medical Association. STEVEN A. CARTER D.C. has been in private practice with Carter Chiropractic in Louisville since 1985. He earned his D.C. degree from Palmer College of Chiropractic and his B.A. degree from Western Kentucky University. Dr. Carter received his certification in peer review, insurance consulting and independent chiropractic examination from the Logan College of Chiropractic in St. Louis, Missouri. He has instructed the program "Herniated Discs: New Guidelines & Language - The Role of MRI in Bodily Injury," sponsored by the Louisville Bar Association in November 2009. Dr. Carter serves on the Aetna Credentialing Committee and the Utilization Review Committee in Louisville, Kentucky. VICKI DANSER is a legal nurse consultant and manager of Legal Nurse Services, LLC. She has 22 years of nursing experience, including writing policies and procedures in the OR; sterile processing, infection control and employee health; and quality assurance chart review. Ms. Danser is a member of the National Alliance of Certified Legal Nurse Consultants (NACLNC) and Association of Operating Room Nurses (AORN). She earned her B.S. degree from University of Kentucky College of Nursing, her B.A. degree from University of Louisville and certification from the Certified Legal Nurse Consulting Program in Las Vegas. MARTHA MARIE EASTMAN is the principal of Eastman Law Office, where she practices in the areas of medical malpractice, nursing home abuse and neglect, general civil litigation, and personal injury. Ms. Eastman was recently a faculty member at the National Judicial College in Reno, Nevada and Trial Advocacy College, where she spoke on essentials of civil litigation sponsored by ATLA. She has spoken at the American Trial Lawyers Association's National Convention in San Francisco, California on "Discovery in Nursing Home Litigation." Ms. Eastman has lectured on nursing home topics in Toronto, Florida, New Mexico and Kentucky. She is a member of the Kentucky, Tennessee and North Carolina bar associations; The Association of Trial Lawyers of America (chair, Nursing Home Litigation Practice Group); the North Carolina Academy of Trial Lawyers; and the Kentucky Academy of Trial Attorneys (Board of Governors). Ms. Eastman earned her B.S. degree from Kent State University and her J.D. degree, cum laude, from the University of Louisville. VIVIANNE GRIFFITHS is an experienced adult nurse practitioner with more than 30 years experience in health care and nursing. Her background includes positions as a staff RN, case manager and director of nursing. Ms. Griffiths' experience and qualifications include: work as a primary care provider for cancer patients, those with mental health needs, adults and adolescents, and family health services. She has worked at Louisville Metro Dept. of Public Health and Wellness Tuberculosis Clinic as a part-time provider and at Paul DuValle Community Health Center-Newburg as a per diem primary care provider. Ms. Griffiths' publications include "The Role of the Advanced Practice Nurse in Hospice and Palliative Care" in the NBNA News (National Black Nurses Association) and "More" ...Community partners help Kyanna B.N.A. support nursing students and award black nursing leaders" in NBNA News. She has been a co-educator with the medical staff in the University of Louisville Palliative Medicine Program, as well as a member of the Hospice Ethics Committee. Ms. Griffiths was named 1990 Nurse of the Year by the Kentucky Nurses Association and 1996 Outstanding Nurse of the Year by the KYANNA Black Nurses Association. She is a member of the American Academy of Nurse Practitioner's, Kentucky Coalition of Nurse Practitioners and Nurse Midwives, National Black Nurses' Association, and the local chapter of KYANNA Black Nurses' Assoc. and the National Alliance of Certified Legal Nurse Consultants. Ms. Griffiths earned her M.S.N. degree from the University of Louisville, Graduate School of Nursing; her B.S.N. degree from Hunter College; and Legal Nursing Consultant Certification from Vickie Milazzo Institute. KAYE MILLER is the owner of North Star Legal Nurse Consulting. Her previous experience is at Commonwealth Health Corporation as a staff RN in the cardiac cath lab, ambulatory surgery and newborn nursery. Ms. Miller has strong clinical experience including Clinical Ladder Level III advanced nursing practice. She is the chairperson of Standards and Practice Council and a member of Leadership Council to obtain MAGNET status (Commonwealth Health Corp). She is presently a sitting member of the Standards Practice Council and was a Nurse of the Year nominee in 2009. Her publications include: Legal Nurse Consulting Ezine, Vol 18, No. 16; "Stand and Deliver," Nursing Notes; "Speak Up," September 2008, Commonwealth Health Corp.; and Nursing Notes; "2009 Magnet Conference," November 2009, CHC. Ms. Miller is a member of the Kentucky Nurses Association, American Association of Legal Nurse Consultants, American Society of Peri-Anethesia Nurses and Certified American Board of Peri-Anesthesia Nurses. She has experience in deposition and trial participation for numerous trials from both the plaintiff and defense perspective. Ms. Miller earned her Legal Nursing Consultant Certification from Vickie Milazzo Institute and is presently attending Western Kentucky University for her B.S. degree. DR. RICHARD T. HOLT is a noted spine surgeon practicing with Spine Surgery, P.S.C., in Louisville, Kentucky. His practice serves as a regional referral center for complex spine reconstructive cases. He performs more than 400 instrumented spine surgeries annually. Dr. Holt specializes in adult and pediatric deformity, adult degenerative disc disease, non-fusion technologies, fractures and tumors. He has published more than 25 publications, in addition to numerous other articles and book chapters concerning spine surgery, and he is a frequently invited speaker at spine surgery seminars around the world. Through the years, Dr. Holt has served on the faculties of the University of Louisville, the University of Kentucky and Tulane University in New Orleans. He currently directs a training program in spine surgery and his fellows for the last 26 years now practice worldwide. Dr. Holt is an American Board of Orthopaedic Surgery board certified spine surgeon and an American Board of Spine Surgery Diplomate. He received his undergraduate degree from the University of Tennessee, Knoxville, and his medical education from the University of Tennessee, Memphis. Dr. Holt is a member of numerous medical societies including the American College of Spine Surgery, the American Medical Association, the Clinical Othopaedic Society, the Kentucky Medical Society, the Kentucky Orthopaedic Society, the Kentucky State Medical Association, the Scoliosis Research Society, the Spine Society of Australia and the North American Spine Society.
