DR. SHIRLEY A. CONIBEAR, MD, MPH, CIME, is highly published and renowned in the field of occupational medicine with more than 35 years of experience. Presently she provides consulting medical director services to such firms as AT&T, USG Corp. and Akzo Nobel. She maintains an active clinical practice in occupational medicine, and performs and oversees medical surveillance examinations to evaluate fitness-for-duty. She does peer reviews for workers' compensation cases and short and long term disability assessments. Dr. Conibear is board certified in occupational medicine and residency trained in family medicine. She also holds a Masters of Public Health (MPH) degree from the University of Illinois. She is a Certified Independent Medical Examiner (CIME) by the American Board of Independent Medical Examiners. Currently, she holds an appointment as adjunct associate professor at the University of Illinois School of Public Health where she provides clinical and didactic training for residents in occupational medicine. DR. GARY KRONEN is a hand surgeon at Hand & Orthopaedic Surgery Centers, LTD. Dr. Kronen is board certified in hand surgery and plastic surgery. He attended medical school at the State University of New York. He completed a fellowship in hand surgery at Baylor College of Medicine in Houston, Texas. Dr. Kronen also completed a microsurgery fellowship at the University of Louisville in Louisville, Kentucky. His plastic surgery residency and general surgery residency were both completed at Montefiore Medical Center in Bronx, New York. DR. G. KLAUD MILLER is a board-certified orthopedic surgeon and the medical director of Windy City Orthopedics and Sports Medicine on the north side of Chicago for almost 30 years. He practices at Swedish Covenant Hospital in Chicago, St. Francis Hospital in Evanston, Illinois and the North Shore Surgicenter in Lincolnwood, Illinois. His practice includes all aspects of orthopedics except spine surgery. His subspecialties include all aspects of sports medicine/arthroscopy as well as joint replacement, hand/foot and trauma surgery. Dr. Miller received his medical degree from Northwestern University Medical School as a member of the six-year honors program in medical education, and completed his residency training there as well. He completed a fellowship in sports medicine, arthroscopy and knee reconstruction in Los Angeles under the direction of Drs. Martin Blazina and James Fox, and was one of the first fellowship-trained sports medicine specialists in the Chicago area. He has published multiple papers in scientific journals and presented his work at numerous national orthopedic meetings. DR. MARK SOKOLOWSKI is an orthopaedic surgeon with Trinity Orthopaedics. Dr. Sokolowski has specialized training in spine surgery. He completed medical school at Rush University, and completed his resident in orthopaedic surgery at Northwestern University in Chicago. He received his fellowship training in adult and pediatric spine surgery at Twin Cities Spine Center in Minneapolis, MN. In addition to his surgical training, Dr. Sokolowski has lectured and published on the treatment of spinal diseases and disorders. His orthopaedic surgery review book was published in 2009. TRACY V. BEDFORD, CRNA, MSN, CLNC, of MedLaw Consulting, Inc. is an advanced practice certified registered nurse anesthetist (CRNA). Ms. Bedford has 16 years' total nursing experience, 11 years as a CRNA and 5 years in the cardiovascular ICU and heart failure unit. She is actively involved as a region director and board member of the Illinois Association of Nurse Anesthetists. Ms. Bedford enjoys working with attorneys and insurance professionals on medical cases and also educating nurses on how to prevent medical errors, maximizing patient safety. DIANA CAFI, BSN, RN, CLNC of CAFI, A.D., & Associates, Legal Nurse Consultants, L.L.C., is a Bachelor of Science in Nursing (BSN) prepared registered nurse. Ms. Cafi is a certified legal nurse consultant and has advance training and certifications. She has more than 17 years of experience working in the health care industry. As a registered nurse, she has established a diverse background in telemetry, psychiatry, medical-surgical, gastro-intestinal recovery and rehabilitation. Ms. Cafi is an elected committee member of Congress on Health Policy and Practice of the Illinois Nurses Association, and serves on the Continuing Education Peer Review Panel of the Illinois Nurses Association. She is currently a professional member of Sigma Theta Tau International Honor Society of Nursing ®, the American Nurses Association, the Illinois Nurses Association and the National Alliances of Certified Legal Nurse Consultants ®. Ms. Cafi will obtain her dual degree of master's of science in nursing and master's in health administration in the summer 2011. JOYCE B. MARTER, LCPC, has been in private practice as a Licensed Clinical Professional Counselor (LCPC) since 1998 and specializes in treating mood disorders, anxiety, self-esteem concerns, relationship issues, infertility and addiction. She has extensive experience providing psychotherapy for individuals, couples and groups coping with mental health issues, relationship challenges and chemical dependency. Ms. Marter graduated from The Ohio State University, with honors, and earned a Masters of Arts in Counseling Psychology from Northwestern University. She was awarded "Distinguished Alumni of the Year" by the Masters in Counseling Psychology Program at Northwestern University in 2008. Ms. Marter has extensive public speaking experience and has been quoted in Crain's Chicago Business and Red Eye Chicago, and was interviewed as a psychological expert by WGN CLTV and ABC's Live Well HD Network. She was selected by Crain's Chicago Business for the 40 Under 40 Class of 2010. MODERATOR - JAMES M. HOFERT is a partner in the Chicago law firm of Hinshaw & Culbertson LLP. Mr. Hofert is an experienced trial lawyer who has a substantial practice in the areas of complex tort litigation and environmental law, including construction injury, bad faith, product liability, toxic tort and medical malpractice. His cases have involved inorganic acids (sulfuric and hydrofluoric acid) and caustic soda, and defense of environmental law cases involving chlorinated hydrocarbons (chlorinated solvents, wood preservatives) brought under CERCLA proceedings, various state environmental statutes and common law actions. Mr. Hofert has defended and tried a good number of medical malpractice cases with significant favorable results and has defended cases in both federal and state courts, with past and pending jurisdictions including Illinois, California, Missouri, Kansas, Colorado, Texas, Indiana and Louisiana. He has gone to verdict in jury trials in more than 80 lawsuits, many of which have been cases involving bad faith. In addition, Mr. Hofert has written and presented for numerous organizations. He earned his B.A. degree from Grinnell College and his J.D. degree, cum laude, from Chicago-Kent College of Law.
